2026 LASM Sponsorship Form

The Louisiana Academy of Sleep Medicine is a not-for-profit, state-wide organization of professionals involved in the treatment and evaluation of patients with sleep disorders. It has the following mission statement: To further the understanding of sleep and sleep disorders, to provide a forum for communication and education of sleep professionals, and to promulgate the highest standards for polysomnography and the clinical diagnosis and treatment of sleep disorders, for the people of Louisiana.

In keeping with this mission, we host an annual meeting of the members which provides a forum for sleep disorders education and exchange of new ideas and technology. We are seeking sponsors at the following levels: Platinum Sponsor $2500.00, Gold Sponsor $2000.00, Silver Sponsor $1500.00, Bronze Sponsor $1000.00, & Product Showcase during lunch (cost of A/V & lunch)

The Louisiana Academy of Sleep Medicine greatly appreciates sponsor level support; however, we also provide an exhibit area at the meeting with tables available on a first come first serve basis for display of your products or services. The cost of a display table is $600.00 for the two days. This year we are offering break sponsorship for those exhibitors that are unable to attend in person. Break sponsors can send us their handouts and information flyers to pass out to participants during the lecture breaks.
